10月8日は「世界視力デー(World Sight Day)」、10月10日は「目の愛護デー」。スマートフォンなどで画面を見る時間が増え、子どもや若者の近視が増えている昨今、目を健康に保つには、何を心掛ければよいのだろうか。慶應義塾大学医学部眼科学教室、南青山アイクリニック東京に所属する四倉絵里沙医師に、近視の原因や予防法を聞いた。

Do you own a pair of glasses? There’s a good chance that you do. More than 60% of all junior high school students in Japan have myopia. For high schoolers, the number rises to over 70%.
One reason is that Japanese kids are spending more time on screens than ever before. They look at screens to check social media, message friends and pass the time on trains and buses. The whole world is on their screens.
That’s very convenient. But ophthalmologist Dr. Erisa Yotsukura says loads of screen time is bad news for your eyes. Yotsukura works in the Department of Ophthalmology at Keio University School of Medicine and at Minamiaoyama Eye Clinic Tokyo. She treats children and young people. “Myopia is appearing at younger ages,” says Yotsukura. “Before, it used to be upper elementary school students who got myopia. Now, even some first- and second-grade elementary school students are getting it.”
Yotsukura says that her young patients have trouble reading the blackboard in school. “The majority of them are fine until third grade. Third grade is when kanji characters on the blackboard suddenly start to look blurry. By sixth grade, some students are in classes where more than half of their classmates wear glasses.”
Why do people get myopia? “There are two big reasons. One is genetics. Myopia is often passed down from parent to child. The other is lifestyle. Kids spend too much time on screens,” says the doctor. She adds: “A good way to prevent myopia is to spend time outdoors. Get some fresh air and sunshine.” Vitamin D, which is formed from the sun’s rays, is very important to the eyes. “Joining a sports bukatsu is great. But even going for a walk on a cloudy day is good. Standing in the shade and looking at something far away is also good,” she says.
Alternatively, Yotsukura says that a method called “30/30/30” can prevent myopia. “First, put at least 30 centimeters between your eyes and your device. Next, take a 30-second break after looking at a screen for 30 minutes. Let your eyes rest.”
How can you rest your eyes? The doctor says: “Roll your eyes around, look out at something far away or put a warm compress over your eyes. For children, taking a 30-second break from a screen will reset their vision instantly.”
Once you do get myopia, however, say hello to glasses and contact lenses. “Luckily, people have a lot more choices now to assist their vision,” says Yotsukura. “For example, multifocal soft lenses correct vision and slow myopia as you wear them. Orthokeratology, which uses special lenses worn at night, helps correct your vision while you sleep. Ten years ago, people did not think myopia was a serious health problem. But now, myopia prevention among children and teenagers is becoming a priority.”

眼鏡を持っているだろうか。おそらく持っている人は多いだろう。日本の中学生の60%以上が近視である。高校生では、その割合が70%を超える。 その理由の一つは、日本の子どもたちがこれまで以上に長い時間、画面を見ていることだ。SNSをチェックしたり、友達にメッセージを送ったり、電車やバスの中で時間をつぶしたりするために、画面を見ている。世界中の情報が画面の中にある。 とても便利なことだが、眼科医の四倉絵里沙先生は、画面を長時間見ることは目によくないと話す。四倉先生は、慶應義塾大学医学部眼科学教室と南青山アイクリニック東京に勤務し、子どもや若者を診療している。「近視になる年齢が低くなっています」と四倉先生は言う。「以前は小学校高学年の児童に近視が多かったのですが、今では小学1、2年生でも近視になる子がいます」 四倉先生によると、子どもの患者には学校で黒板の文字を読むのに苦労する子がいる。「小学3年生までは、ほとんどの子は問題ありません。3年生になると、黒板に書かれた漢字が突然ぼやけて見えるようになります。6年生になると、クラスの半分以上が眼鏡をかけているという学校もあります」 なぜ近視になるのだろうか。「大きな理由は2つあります。一つは遺伝です。近視は親から子へ受け継がれることがよくあります。もう一つは生活習慣です。子どもたちは画面を見すぎています」と先生は話す。さらに、「近視を予防するには、屋外で過ごすのがよい方法です。外の空気を吸って、太陽の光を浴びましょう」と話す。太陽の光を浴びることで体内で作られるビタミンDは、目にとってとても重要である。「運動部に入るのもいいですね。でも、曇りの日に散歩するだけでもいいです。日陰に立って遠くのものを見るのもいいですよ」と先生は言う。 別の方法として、四倉先生によると、「30・30・30」という方法でも近視を予防できる。「まず、目と機器の間を30センチ以上離します。次に、30分間画面を見たら、30秒間休憩します。目を休ませましょう」 では、どうすれば目を休ませることができるのだろうか。先生は「目をぐるぐる動かしたり、遠くのものを見たり、目に温かい蒸しタオルなどを当てたりしてください。子どもなら、画面から30秒離れるだけで、すぐに視力がリセットされます」と話す。 しかし、いったん近視になると、眼鏡やコンタクトレンズのお世話になる。「幸い、今は視力を補うための選択肢がたくさんあります」と四倉先生は言う。「例えば、多焦点ソフトコンタクトレンズは、装用することで視力を矯正し、近視の進行を抑えます。オルソケラトロジーは、夜間に特殊なレンズを装用することで、寝ている間に視力を矯正する方法です。10年前は、近視が深刻な健康問題だとは考えられていませんでした。しかし今では、子どもや10代の若者の近視予防が優先事項になっています」
